The Church Permission and Medical Release Form is a required form for all participants attending the girls' camp. Please click the link below, fill out the information, and print the file. If you are an adult attending, sign for yourself. If you are a youth attending, please have your parent sign the form. The form is to be emailed or given to the ward Young Women Leaders.
